Information this website collects
When you submit the contact or appointment request form, we collect the name, phone number, email address and any details you choose to include. We use that solely to respond to you and to schedule care.

Your rights under HIPAA
The practice’s Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can get access to it. It covers, at minimum:
