Your first visit
Everything you need before you arrive — forms, insurance, what to bring, and what the appointment will actually be like.

Book
Request online or call the office nearest you. Most new patients are seen within a week.

Prepare
Complete your forms at home and bring your insurance card, ID and a list of medications.

Evaluate
A full history, hands-on exam and, where needed, in-office digital X-ray — usually in the same visit.

Plan
You leave with a written plan: what it is, what we are doing about it, and what happens next.
Bring with you
- Photo ID and your insurance card
- A list of current medications and dosages
- Referral or prior imaging, if you have them
- The shoes you wear most — they tell us a lot
- Completed new patient forms
